To request a certificate re-issue please contact us on 0151 494 2072 before completing the Learner Replacement Certificate Request form. We will check the database to ensure you are eligible for a replacement certificate and will provide you with your registration number and details of your course which are required when completing the form.
Please Note: Certificates cannot be re-issued in another name. No changes can be made to the original name on the certificate after achievement.
You will be expected to provide a form of ID when requesting a re-issue. A photocopy of one of the following is acceptable to verify identity:
- Birth Certificate
- Driving Licence (either photo or paper)
- NHS card
- Identity card
- Valid EU photo ID card
A replacement certificate will be issued within 20 working days of receipt of all authenticated claims. A charge will be made for each replacement certificate of £25. For more information please see our Fees & Charges leaflet. You can either pay by card, cheque, online banking (details below), or postal order (cheques should be made payable to Open Awards).
For Cheques/Postal Orders, please send a copy of the Learner Replacement Certificate Form and your ID to: Open Awards, 17 De Havilland Drive, Estuary Commerce Park, Speke, LIVERPOOL, L24 8RN.
For Internet Banking payments you will need to email the completed Learner Replacement Certificate Request Form and a scanned copy of your ID to: firstname.lastname@example.org.
Please make all Internet Banking payments to: Sort Code: 01-01-97, Account Number: 21078025 and quote your name in the ‘Reference Box’.
