Replacement Certificate
To request a certificate re-issue please complete the Provider Replacement Certificate Form and return it to us. You will need to return any incorrect certificates before we can process the reissue.
The completed Provider Replace Certificate Form can be sent to reissues@openawards.org.uk or you can send it, along with any incorrect certificates, to our postal address:

A replacement certificate will be issued within 20 working days of receipt of all authenticated claims. A charge will be made for each replacement certificate: Fees & Charges.
